A D4 medical is an evaluation where a doctor fills out a D4 form to assess the fitness of individuals to drive vehicles like HGVs,
LGVs, PCVs, and ambulances. The purpose of this assessment is to ensure that drivers meet the necessary health
requirements for safe vehicle operation.

To apply for or renew your Group 2 license, you need to complete a D4 medical assessment, which must be done by a GMC-registered
doctor. We have GMC-registered doctors, specially trained to perform DVLA medicals.
While your GP can conduct the D4 medical, it is often quite costly (typically over £100). Alternatively, MediDrive offers the same service, performed by a fully trained GMC-registered doctor, for just £50. A D4 medical form is valid for 4 months from the date it is signed by the doctor. You must submit it to the DVLA within this period, or it will expire, and you’ll need to book and pay for a new medical.

The D4 medical process begins with an initial assessment, followed by another at age 45. After that, assessments are required
every 5 years until the age of 65, at which point they must be done annually.

The D4 medical costs £50.

Please bring your driver’s license or passport for identification, a list of any medications you are taking, your glasses prescription (if applicable), and any relevant medical records. We have all the necessary forms available in our office.

If your license has expired, you won’t be allowed to drive until the DVLA issues a new one. If your license is still valid, you’ll need a check-up with your GP, a new D4 medical exam, and to reapply under Section 88. Where appropriate, we offer a free re-test if needed.
